If you are planning on coming to Ukraine to give the sperm sample:

• Before the trip we would ask for both for your passports
• Marriage certificate with an apostille
• Medical certificate to confirm the need for the surrogacy (4 failed IVFs, loss of uterus, any other major factor that prevents you from carrying or conceiving a child) (only for the Intended Mother)
• Spermogram (with motility and morphology), HIV, hepatitis B and C, syphilis (only for the Intended Father) (optional, as we can also do these analyzes here)

If you have decided to send us your material and avoid additional trip to Ukraine at this point, we will ask you for:

• Notarized copies of your passports with apostilles
• Marriage certificate with an apostille
• Medical certificate to confirm the need for the surrogacy (4 failed IVFs, loss of uterus, any other major factor that prevents you from carrying or conceiving a child) (only for the Intended Mother)
• Spermogram (with motility and morphology), HIV, hepatitis B and C, syphilis (only for the Intended Father)
• Power of attorney (we will send you the template) that both of you will have to sign and put an apostille on it. This document will empower us to sign necessary medical documents for the clinic here as well as the surrogacy contract that has to be signed before notary.

Notes:

• The originals of all of these documents will have to be forwarded to us by DHL in case you will not come to Ukraine for the start of the program.
• What is an apostille. The Apostille is a legal regime of recognition of the legal validity of documents to be used abroad. The Apostille came as an alternative and universal certification procedure for documents, issued in one country, but beyond its borders.

• How to send to us your material. We are working with a Ukrainian company that provides a service of sending biological material from one country to another. Please, contact us directly to know more about how to get in touch with this company and its prices for transportation from your country.
